    Default CC: "Fever with pain of inferior abdomen, groin, and scrotum"

    HPI: A 18 yrs old male sought for hospital due to fever with pain of inferior abdomen, groin, and scrotum. The patient reported fatigue, fever, chilling, later along with headache, vomiting, diarrhea, poor appetite, waist pain, and difficult of urination. He had these conditions for 4 days.

    PMHx: Exercise-induced asthma; trauma of testis and testis pain 3 weeks ago, which relieved 30 minutes later.

    Hospitalizations: Hospitalization due to diarrhea.

    Soc Hx: smoking (? pack-yrs); Marijuana user, 3 times per week; intermittent ingestion of alcohol; no dissolute sexual activity.

    Vitals: T 39.4C; BP 125/67 mm Hg; HR 121 bpm; RR 36 per minute; SaO2 100%.
